Under the Individuals with Disabilities Education Act (IDEA), Transition Services are a protected right for students age 16 and older who qualify for Special Education. 


These services include assessment, planning, and the development of an Individual Transition Plan (ITP) as part of a Free Appropriate Public Education (FAPE).


Yet when high school ends, many of these supports fall away, leaving young adults and their families without structure at one of life’s most pivotal transitions.
