Using evidence-based approaches, including the UCLA PEERS® social skills framework, we help students build confidence, strengthen resilience, and take meaningful steps toward their goals in education, work, and life.
The Transition Planning Process
Our process is structured, individualized, and designed to create momentum.
1. Initial Assessment and Planning
Comprehensive interest inventory and strengths-based evaluation.
2. Interpretation
Translation of results into aligned opportunities across education, career, and independent living.
3. Weekly Coaching Sessions
Personalized instruction targeting executive functioning, social-emotional growth, and readiness skills.
4. Person-Centered Plan Development
A clear, actionable roadmap for education, employment, and independent living.
Deliverable: Individual Transition Plan (ITP)
Each client receives a personalized plan that:
Defines strengths, goals, and next steps
Provides practical strategies and tools for immediate use
Coordinates supports across education, career, relationships, and independence
This is a working roadmap for real-life progress.
