Our Signature Program for Young Adult Transition Services at Celo Learning 

Transition Services are a protected right under the Individuals with Disabilities Education Act (IDEA) for students aged 16 and older who qualify for Special Education. These services must include assessment, planning, and the development of an Individual Transition Plan (ITP) as part of a Free Appropriate Public Education (FAPE).

Celo Learning offers private ITP  services that complement and extend far beyond what is typically provided in public school settings.  

Our approach is personalized, future-focused, and designed to empower students and families with the tools, insights, and support needed to navigate the transition to adulthood with confidence and clarity.

Initial ITP Assessment & Planning

$2,900.00 | Timeline: 60 Days

Our Individual Transition Plan (ITP) process begins with a comprehensive, strengths-based evaluation. 

Over the course of 8 weeks, clients participate in:

• Direct consultation and coaching

• Weekly planning meetings

• Neurodiversity-affirming assessments

• Tailored recommendations aligned with your goals

The result is a fully individualized roadmap designed to support successful transitions across life, learning, and work.

Celo Gateway

Upon completion of your Initial ITP, clients ages 18–35 gain access to CELO GATEWAY—a curated menu of à la carte services that support education, employment, relationships, and life transitions.

Whether planning for college, building executive functioning skills, or preparing for the workplace, CELO GATEWAY connects young adults with trusted providers and empowering tools to build the future you want—at a cost you can expect.
